Permission to Disclose Medical Information
During the week’s hectic schedule, we realize that things come up that make it impossible for you to keep your scheduled appointment time. Please let us know in advance if you are unable to keep your appointment. Because of our many waiting patients, we have been compelled to implement a $50 charge on appointments that are not cancelled 24 hours prior to the scheduled appointment time.
